In 2010, through exclusive access to prison video, The Fifth Estate uncovered details of inmate Ashley Smith’s treatment by federal prison guards and the moments leading up to her death by suicide.
Smith was a troubled 19-year-old when she died at Grand Valley Institution in Kitchener, Ont. Her death made national headlines and led to a scathing report by Canada’s federal prison ombudsman. The Fifth Estate fought for video showing Smith being pepper-sprayed and subdued by physical force and put into a restrictive body bag called “the wrap.”
In 2013, an Ontario coroner’s jury ruled Smith’s self-inflicted death in her segregation cell a homicide and made a series of recommendations to prevent similar deaths.
The Fifth Estate is marking its 50th year of investigative journalism. Watch the documentary Out of Control and more from the show’s files on YouTube.
