On April 26, 1986, a late-night safety test at the Chernobyl Nuclear Power Plant in Soviet Ukraine went catastrophically wrong. A combination of critical human errors and a flawed reactor design led to the worst nuclear disaster in history, releasing massive radioactive contamination across Europe.
